What Is an Aortic Aneurysm?

An aortic aneurysm is an abnormal bulge that forms in the wall of the aorta, the body’s largest artery, when the vessel wall weakens and stretches outward. A segment of the aorta is generally considered aneurysmal once it measures at least 50% wider than its normal diameter.1
Aortic aneurysms are classified primarily by location. Abdominal aortic aneurysms develop in the portion of the aorta that runs through the abdomen, while thoracic aortic aneurysms develop in the portion that passes through the chest, and each type carries somewhat different risk factors and treatment considerations.2

A widened portion of the aorta qualifies as an aneurysm if it’s at least 50% wider than the normal aortic diameter.

Types of Aortic Aneurysms

Thoracic aortic aneurysms are further classified by which part of the chest aorta they affect. About 60% of all thoracic aortic aneurysms involve the ascending aorta, the segment closest to the heart, while others affect the aortic arch or the descending thoracic aorta.2
Abdominal aortic aneurysms, covered in more detail on our dedicated abdominal aortic aneurysm page, are generally more common overall and are strongly associated with smoking history and atherosclerosis, while thoracic aneurysms are more often linked to genetic connective tissue conditions and long standing high blood pressure.3

Causes

Aortic aneurysms form when the aortic wall is weakened by a combination of factors, including atherosclerotic plaque buildup, chronic high blood pressure, and, in some cases, underlying connective tissue disorders that affect the strength of the vessel wall from birth.3
Additional contributing factors include male sex, smoking, elevated cholesterol, inflammation of the blood vessels, and, less commonly, untreated infections that damage the aortic wall directly.3

Symptoms

Aortic aneurysms, particularly smaller ones, frequently cause no symptoms at all and are often discovered incidentally during imaging performed for another reason.1
When thoracic aneurysms do cause symptoms, they can include difficulty swallowing, hoarseness, or signs resembling heart failure, such as cough, shortness of breath, or leg swelling, all resulting from the enlarged aorta pressing on nearby structures.2

Risk Factors

Risk factors shared across most aortic aneurysms include older age, smoking, high blood pressure, and a family history of aortic disease. Thoracic aneurysms are additionally linked to inherited connective tissue disorders, which can weaken the aortic wall independent of typical cardiovascular risk factors, meaning some patients develop aneurysms at a younger age than typically expected.3

Complications

The most serious complication of any aortic aneurysm is rupture, in which the weakened vessel wall tears, causing severe internal bleeding that requires emergency treatment. Aortic dissection, a related but distinct condition in which the layers of the aortic wall separate, is a particular concern with thoracic aneurysms and can occur even before an aneurysm reaches a size that would typically prompt elective repair.1,2

Treatment

Small, asymptomatic aneurysms are generally monitored with regular imaging to track growth over time, alongside medical management of blood pressure and cardiovascular risk factors.1
Larger or symptomatic aneurysms typically require repair, either through open surgery, in which the weakened section of aorta is replaced with a graft, or through endovascular repair, a minimally invasive approach in which a stent graft is guided through the arteries using a catheter to reinforce the aneurysm from within.4

Prevention

While genetic risk factors cannot be changed, modifiable risk factors can meaningfully reduce the likelihood of aneurysm formation or progression. These include not smoking, controlling blood pressure, managing cholesterol, and attending recommended imaging or screening appointments, particularly for people with a family history of aortic disease.3
