What Is Breast Augmentation?
Breast augmentation is a surgical procedure that increases breast size and can improve breast shape using implants or, in some cases, a patient’s own fat. It’s consistently one of the most performed cosmetic surgical procedures in the United States, sought by patients for a range of reasons, including personal preference, correcting asymmetry, or restoring volume lost after pregnancy or weight change.1
Most breast augmentations use implants, silicone shells filled with either saline solution or silicone gel, though fat transfer augmentation, which uses a patient’s own fat cells harvested from another part of the body, has become a more common alternative in recent years.1,2

Who Is a Candidate for Breast Augmentation?
Good candidates are generally in good overall health, both physically and psychologically, with realistic expectations about the outcome. Surgeons evaluate breast tissue quality, skin elasticity, and existing breast volume to determine the most suitable implant type, size, and placement.1
For fat transfer augmentation specifically, candidates need enough donor fat elsewhere in the body, since the surgeon must remove more fat cells than will ultimately be used, as a portion of transferred fat does not survive the transfer process.3
Implant Types

Saline implants are filled with sterile salt water after being placed in the body, allowing for a smaller insertion incision. If the implant shell leaks, the saline is safely absorbed and expelled by the body naturally.1
Silicone gel implants are prefilled with a cohesive silicone gel designed to feel closer to natural breast tissue. According to the FDA, available data show no apparent association between silicone gel implants and connective tissue disease, breast cancer, or reproductive problems, and the agency considers them to have a reasonable assurance of safety and effectiveness when used as labeled.4
Structured saline implants contain an inner structure intended to make a saline filled implant feel more natural, combining some benefits of both saline and silicone options.1
The Procedure
Breast augmentation is typically performed under general anesthesia or intravenous sedation, depending on the surgeon’s approach and the patient’s preference. Common incision locations include along the lower edge of the areola, within the crease beneath the breast, or in the armpit, each chosen based on implant type, size, and the patient’s anatomy.1
Once the incision is made, the surgeon creates a pocket either above or below the chest muscle to hold the implant, positions the implant, and closes the incision. Fat transfer augmentation instead involves liposuction to harvest fat from areas such as the abdomen or thighs, processing of the fat, and injection into the breast tissue in multiple small amounts to support even distribution and survival of the fat cells.2,3
Recovery
Most patients experience swelling, soreness, and tightness in the chest for the first one to two weeks following surgery, with many returning to non strenuous work within about a week. Strenuous exercise and heavy lifting are generally restricted for four to six weeks to protect healing tissue.1
Breast implants are not considered lifetime devices. Patients are generally advised that they may need replacement surgery approximately every 10 to 15 years, whether due to implant rupture, changes in personal preference, or normal wear over time.1
Risks and Complications
Most people with breast implants do not experience serious complications, but risks exist and should be discussed thoroughly with a surgeon. Capsular contracture, in which scar tissue around the implant tightens and can cause firmness or pain, is among the more common complications.1
Breast implant associated anaplastic large cell lymphoma, a rare cancer of the immune system linked most strongly to certain textured implant surfaces, is a risk the FDA has specifically investigated, leading to the removal of some textured implant models from the market. Some patients also report a cluster of systemic symptoms sometimes referred to as breast implant illness, an area of ongoing research.5
Based on currently available evidence, implant fill type, saline or silicone, does not appear to be a specific risk factor for this lymphoma, though the FDA notes this has not yet been evaluated in a large, dedicated study.5
Results
Results are generally visible once initial swelling subsides, typically within a few weeks, with final results apparent by three to six months as tissue settles fully. Because implants are not permanent devices and natural aging continues to affect the breasts and surrounding tissue, most patients should expect the possibility of future revision surgery over their lifetime.1
