What Is Fat Grafting?

Fat grafting, also known as autologous fat transfer, lipofilling, or fat injection, is a procedure in which a surgeon removes fat from one area of the body through liposuction and injects it into another area to add volume or improve contour. Because the fat comes from the patient’s own body, it’s generally well tolerated and avoids the use of synthetic implant material.1
Fat grafting is used in a wide range of applications, including breast augmentation and reconstruction, buttock augmentation, facial rejuvenation, and refining the results of body contouring procedures such as liposuction.1,2

Who Is a Candidate for Fat Grafting?

Good candidates are generally in good overall health, do not smoke, and are able to maintain a stable weight, since drastic weight changes after the procedure can affect the survival and appearance of the transferred fat.1
Patients need an adequate donor site with enough fat to harvest, since more fat must be removed than will ultimately be used, as a portion of transferred fat cells do not survive the process. This means people with very little body fat may not be ideal candidates for fat grafting.1

The Procedure

The harvested fat is then processed, or conditioned, to separate viable fat cells from blood, fluid, and damaged cells. Finally, the purified fat is carefully injected into the target area in small amounts, often through multiple passes, to maximize the fat’s blood supply and chances of survival.1
Depending on the treatment area and extent of correction, the procedure can be performed under local anesthesia with sedation or general anesthesia, and typically takes one to several hours.

Recovery

Recovery involves healing at both the donor site, where liposuction was performed, and the area where fat was injected. Swelling and bruising are common at both locations and can take up to several weeks to fully subside.1
Some patients return to work and normal activities within about a week, while others need two weeks or more, depending on the extent of the procedure and how much fat was transferred.1

Risks and Complications

Bruising and swelling are expected following both the fat harvesting and injection steps. Because a portion of transferred fat cells do not survive, the treated area may lose some volume over the following months before results stabilize, and if a significant portion of fat does not survive, it can lead to fat necrosis, firm or lumpy areas, or the formation of oil cysts at the injection site.1
When fat grafting is used in breast procedures, available evidence has not shown that it increases the risk of breast cancer, though long term monitoring and follow up imaging remain important given the relatively newer widespread use of the technique in this area.4

Results

Results become more apparent as initial swelling resolves, typically over several weeks, though the fat continues to settle over the following months as some volume is naturally reabsorbed by the body. Once the surviving fat stabilizes, results are generally considered long lasting, behaving like the patient’s own natural tissue since it is, in fact, living fat cells.1
