What Is Hepatocellular Carcinoma?
Hepatocellular carcinoma is the most common type of primary liver cancer, accounting for up to ninety percent of cases in which cancer originates directly within the liver rather than spreading there from another part of the body.1

Symptoms

Chronic hepatitis B or hepatitis C infection, autoimmune hepatitis, chronic alcohol use, obesity, and diabetes mellitus all represent established risk factors for developing hepatocellular carcinoma.3
In the United States specifically, additional recognized risk factors include being born between nineteen forty five and nineteen sixty five, male sex, Hispanic ethnicity, metabolic syndrome, excessive alcohol use, and smoking.3
Among people with cirrhosis caused by chronic hepatitis B or hepatitis C infection, the annual risk of developing hepatocellular carcinoma ranges from approximately two to five percent, underscoring the importance of regular surveillance in this high risk population.3
Complications
As hepatocellular carcinoma advances, tumors can grow large enough to significantly impair remaining liver function, worsening symptoms related to underlying cirrhosis and liver failure.
In rare but serious cases, tumor tissue can extend directly into major blood vessels, including the veins that carry blood from the liver back to the heart, occasionally reaching as far as the right side of the heart itself.4
Because hepatocellular carcinoma so often develops in the setting of preexisting cirrhosis, patients frequently face the combined burden of both cancer progression and declining liver function, complicating treatment decisions and overall prognosis.
Treatment
Surgical tumor removal, whether through partial liver resection or full liver transplantation, remains the only treatment approach with true curative potential for hepatocellular carcinoma.5
Thermal ablation offers another potentially curative option for carefully selected patients, though its curative potential is generally limited to smaller tumors, typically under two centimeters in size.5
Unfortunately, only about fifteen percent of all hepatocellular carcinoma cases are suitable for surgical treatment, meaning that many patients instead receive noncurative treatments, including intraarterial therapy, radiation, or systemic therapy, aimed at slowing disease progression and preserving quality of life.5

Vaccination against hepatitis B represents one of the most effective, widely available strategies for preventing hepatocellular carcinoma, directly addressing one of its leading underlying causes.1
