Pulmonary arterial hypertension
Overview
Pulmonary arterial hypertension (PAH) is a rare, progressive condition in which the blood vessels in the lungs become narrowed and thickened, raising the pressure required to pump blood through them. Over time, this extra strain can weaken the right side of the heart, which is responsible for pumping blood to the lungs.
Symptoms
Early symptoms are often subtle and can include shortness of breath, particularly during physical activity, and fatigue.
As the condition progresses, symptoms can include chest pain, dizziness or fainting, swelling in the legs and ankles, and a racing heartbeat.
Causes
PAH occurs when the walls of the pulmonary arteries thicken and stiffen, narrowing the space available for blood flow. This can happen without a clear cause (idiopathic PAH), be inherited, or develop as a result of another condition such as connective tissue disease, congenital heart defects, or certain medications.

- Connective tissue diseases, such as systemic sclerosis
- Congenital heart disease
- Family history of PAH
- HIV infection
- Liver disease
- Certain diet drugs and medications
Complications
As PAH progresses, the added strain on the right side of the heart can lead to right heart failure, a serious and life-limiting complication that significantly affects quality of life and long-term outlook without treatment.
